What is an HMO plan? If you’re shopping for health coverage, you’ve probably seen the term come up, but you may not know exactly what it means for you.

An HMO (Health Maintenance Organization) is a type of health insurance plan that limits coverage to a specific network of doctors, specialists, and hospitals. In most cases, you’ll need to choose a primary care physician (PCP) who manages your overall care and provides referrals if you need to see a specialist.

One of the biggest draws of an HMO plan is cost. Because the plan uses a defined provider network, premiums and out-of-pocket costs are often lower than those of other plan types, such as PPOs. The tradeoff is flexibility; HMOs generally won’t cover care outside the network except in a true emergency.

HMOs aren’t limited to individual or employer coverage, either. Many Medicare Advantage plans are structured as HMOs, offering coordinated care through a network for Medicare beneficiaries.

HMO plans have been part of the U.S. healthcare landscape for decades, evolving through several different structural models — group, network, staff, and IPA models — each shaping how care is delivered today.

Who is an HMO plan best for?

HMO plans tend to work well for people who don’t travel frequently, already have a preferred local doctor, and don’t often need to see specialists. Because HMOs emphasize preventive care, routine checkups, screenings, and wellness visits are often covered at little or no additional cost, making it easier to catch health issues early.

Before enrolling, it’s worth confirming that your preferred providers are in-network and understanding the plan’s referral process, so there are no surprises when you need care.
